A key component of the site is our provider profiles, which allow users to research who they want to provide their healthcare. It is vital to have information on our healthcare providers that is accurate, informative, and up-to-date at all times.

To add or update a provider’s information, please fill out the form below as completely as possible. The information provided is used to connect the profile with relevant health conditions, services, locations and is essential to providing patients with complete information to make informed healthcare decisions.

Please note that many fields are required. Please submit the form when you have all the information necessary for the required fields. 

Publications

Publications are automatically imported from PubMed. If there is an error with the publication, please notify the publisher directly so they can correct it, and we will automatically update your profile when we receive a correction from PubMed.

Phone Number

The phone number listed on a provider’s profile is fed from their MyUFL Account. The provider will need to log-in to their MyUFL Account and update their business phone number in their directory profile. This business phone number feeds many other systems, including the main UF Directory, and is available to the public.
